By submitting this form and a photograph and name to Not an Easy Fix Inc. ("NAEF"), you warrant and represent that you have read, understood, and agreed to the following Terms and Conditions for submission of photographs and names;
1) By submitting the photograph, you grant to NAEF a royalty-free, non-exclusive right and license to display, edit, and use the photo and person’s name on NAEF’s memorial website page and at events conducted by or sponsored by NAEF.

2) You are a family member, a spouse, or have obtained permission from a family member or spouse of the deceased; 

3) The person being memorialized – and no one else – is pictured in the photograph; 

4) That substance use directly and substantially contributed to the person’s death, and that you have in good faith provided accurate information.
